The application form, fee and supporting documents for a passport name change :Cross Stitch – Travel and transport

The Identity and Passport Service (IPS) has one standard form for all types of individual applications. The form comes with full instructions, whether you fill in the paper version or complete it online. Learn how to get the form and what fee and supporting documents you need to send IPS.
For an adult passport it costs £72.50 for standard service, £103.00 for the one-week Fast Track service and £128.00 for the Premium one-day service. For a child passport it costs £46.00 for standard service, £87.00 for the one-week Fast Track service and £106.50 for the Premium one-day service.
Only you need to sign the form if you are applying just to change your name. No one else needs to sign it, unless your appearance has changed considerably since your last passport, or your passport is lost, stolen or damaged.
Which name to sign
You should sign the form at section 9 with your new name. Use your new name even if you are waiting to get married or enter a civil partnership and have not yet officially changed your name.
IPS needs to see evidence of all your name changes. You must send original documents, not copies. IPS will accept laminated documents as proof of name changes.
You will need to send:
- completed application form
- your current or last passport
- two recent, identical photographs
and supporting documents as detailed below.
In all other cases, IPS needs to see your change of name deed.
